A practical checklist for wheelchair readiness, departure paperwork, building access, contacts, and the return trip from a facility.

A scheduled ambulette pickup at a residence, rehabilitation center, or other facility involves more than a vehicle arrival time. The passenger, wheelchair, approved documents, belongings, entrance, and responsible contacts all need to be aligned. Clear ownership prevents the transportation crew from arriving before the facility is ready to release the passenger.
Name the person coordinating departure
Identify the facility staff member or family contact who can confirm readiness and answer a transportation question. Provide the appropriate unit, floor, and direct phone number instead of only the building’s main switchboard.
Decide who will bring the passenger to the approved meeting point and who will retain responsibility until the transportation handoff occurs. The exact arrangement depends on the facility and service; it should be confirmed rather than assumed.
Verify the wheelchair and travel equipment
Tell dispatch what type of wheelchair will travel and whether the passenger has leg supports, a specialized seating system, a communication device, a portable oxygen system, or other equipment. Report changes from earlier trips before the vehicle is assigned.
Prepare the device according to its manufacturer and care-team instructions. Do not ask transportation staff to repair equipment, change a clinical setting, or decide whether a medical supply is adequate for the appointment and return.
Gather approved trip information
Keep the destination address, department, appointment time, passenger identification, authorization details, and facility contact together. The facility should determine which documents may accompany the passenger and protect health information through its approved process.
The driver should not be used as an informal messenger for loose clinical paperwork or medication. If information must reach the destination, the sending and receiving organizations should use an authorized handoff method.
Confirm the route from unit to vehicle
Check the elevator, door, hallway, ramp, loading area, and security procedure that apply at pickup time. Reserve an elevator or notify a front desk when the building requires it. Tell dispatch about construction, a broken lift, or a temporary entrance.
A wheelchair-accessible vehicle does not solve an inaccessible building path. The facility should identify an approved route and alert the transportation provider when conditions change.
Plan the return before departure
Confirm whether the return is scheduled, requested after the visit, or arranged by another party. Identify the destination entrance for the return and the person who will receive the passenger at the facility.
